Intakes can be seen as soon as the same day of first contact. A member of our assessment team will schedule an interactive face-to-face or virtual clinical assessment to evaluate the nature of your problems, understand your clinical history and work with you to figure out the best possible next steps. After meeting with an intake coordinator, Healing Bridges will either link you to our specialized services or allow you be redirected to resources that better suit your needs. Get connected to the Intellectual and Developmental Disabilities (IDD) Supports Coordination Program
Please call the one of our IDD/A Supervisors at 724-225-6940 x 503 or 501 and request supports coordination services from Healing Bridges. One of our IDD/A SC Supervisors will then ask a few basic questions about the individual. Once all the pre-screen questions are completed, you will be directed to fill out the Washington County Behavioral Health and Developmental Services IDD/A Referral Form by clicking the button below. If you are a person who does not have computer access to fill out the form, please contact Michelle Gindlesperger at the Washington County BHDS office at 724-228-6833 and request IDD/A supports coordination services from Healing Bridges. If the individual is approved, then a supports coordinator will be assigned.

Get connected to the Blended Case Management (BCM) Program
Please call our BCM Supervisor/Chief Operating Officer (COO) at 724-225-6940 ext. 113 and request blended case management services. Once our services are requested, our BCM Supervisor/COO will help you fill out our BCM Center for Community Resources (CCR) Intake Form to determine eligibility for services. If the individual meets the proper criteria, then a blended case manager will be assigned. For any questions regarding the form or updates in your eligibility status, please contact our BCM Supervisor/COO for assistance.
